For those with diabetes, we offer therapeutic diabetic shoes and continuous glucose monitoring (CGM) equipment to help them stay on top of their blood sugar levels.

Through the PrimaryChoice MTM program, our pharmacists work directly with patients, their providers, and community hospitals to ensure optimal treatment while reducing the risk of adverse events and drug interactions. Performing an accurate medication reconciliation, is a critical element of a successful discharge transition.

Pharmacists have many responsibilities in transitions of care models that reach far beyond medication reconciliation. Our pharmacists ensure that patients have follow-up appointments with appropriate physicians, counsel patients about disease state management and recommended lifestyle changes.
