About us

At Primary Choice Pharmacy, we understand that managing your health can be difficult, especially when you’re dealing with chronic conditions like diabetes. That’s why we’re dedicated to providing a wide range of services and products to help our patients take control of their health and well-being.

As a full-service pharmacy and medical supply provider, we offer everything from dispensing prescriptions and medication therapy management (MTM) to blister packing and immunization services. And with our free home delivery, we make it easy for our patients to access the care they need.

But we don’t stop there. We also offer specialized services like diabetes self-management education (DSME) and medication adherence packaging to help our patients better manage their conditions. For those with diabetes, we offer therapeutic diabetic shoes and continuous glucose monitoring (CGM) equipment to help them stay on top of their blood sugar levels.

We believe that healthcare is a collaborative effort, and that’s why we work closely with healthcare providers to achieve the best possible outcomes for our patients. Our staff of face-to-face consultant pharmacists provide medication and health education, complete care coordination, and patient financial assistance.

At Primary Choice Pharmacy, we see ourselves as a trusted partner in our patients’ healthcare journey. We provide not only traditional prescription medications, but also over-the-counter drugs, supplements and other products to help them stay healthy. Our ultimate goal is to improve patient outcomes by providing the highest quality products and services, and to be an essential resource for our patients and healthcare providers.

We are Delivering Pharmacy…

Medications when you need them, no matter where you are

Primary Choice Pharmacy are “closed door” facility specializing in comprehensive, personalized patient support services, expertise in enhancing medication adherence through patient empowerment and education.

Expertise in MTM clinical services.
Through the PrimaryChoice MTM program, our pharmacists work directly with patients, their providers, and community hospitals to ensure optimal treatment while reducing the risk of adverse events and drug interactions. Performing an accurate medication reconciliation, is a critical element of a successful discharge transition.

Pharmacists have many responsibilities in transitions of care models that reach far beyond medication reconciliation. Our pharmacists ensure that patients have follow-up appointments with appropriate physicians, counsel patients about disease state management and recommended lifestyle changes. They also educate patients about conditions other than those for which they were hospitalized. “Seventy percent of 30-day hospital readmission rates are not the result of an exacerbation of the same disease state”
At PrimaryChoice we’re more than a pharmacy. Our Compounding Center has decades of experience creating customized prescriptions for physicians .
